Clinical Observation on Genital Bleeding during the Climacterium




Abstract
A retrospective study was performed by reviewing the records of 500 perimenopausal women who had visited the Dept. of Obstetrics & Gynecology, Chung-Ang University Hospital because of abnormal vaginal bleeding during the period of May 1st, 1978 to April 30th, 1981. The following conclusions were derived from an overall analysis of the records by age distribution, change of menstrual pattern, causes of vaginal bleeding,
and microscopic finding of endometrium.
1. Women aged from 40-44 years showed the highest frequency in complaining about the abnormal vaginal bleeding, comprising 43.8 of the total and those who were over 55 years of age showed the lowest frequency with 8.6%.
2. The prime cause of perimenopausal vaginal bleeding was dysfunctional uterine bleeding with 20.5%0, followed by myoma (19.29¢¥0), inflammation(19.2%), and pregnancy and it¢¥s complications (9.5%) in order.
3. Analysis of menstrual patterns of 362 patients whose description of menstrual pattern was relatively accurate revealed that bleeding patterns varied depending on the type of diseases, though most of them showed the pattern of metrorrhagia and menometrorrhagia.
4. The endometrial biopsy findings in 48% of 170 cases who did not have any particular organic lesion appeared to be in proliferative¢¥ phase and hyperplasia.
5. In terms of the treatment for the perimenopausal vaginal bleeding, hysterectomy was performed to 199 patients (40%) and D & C to 154 patients (30%), while hormonal therapy and conservative treatment amounted to about 18%.
The menopause is identified by a gradual rise in the frequency of irregular menstruation, dysfunctional uterine bleeding and cervical cancer, and also by increase in frequency of hypertension, cardio-vascular disease, thyroid disease, and emotional disturbance. When treating abnormal genital bleeding of menopausal patients, a thorough analysis of the cause must be performed beforehand and especially a careful examination for the detection of malignancy must precede any treatment,
